A group of local health, social care, voluntary sector and Council professionals have come together to try help improve the health, wellbeing and life chances of the people of Farnham. It is vital that they hear from our community about their experience of life in Farnham, to help identify ways of achieving their goals. 

This is their first survey and they are particularly interested in hearing from people living in North Farnham. At the end of the survey you will be given the opportunity to enter a prize draw to receive a one month family membership at Farnham Leisure Centre.

The survey should take no longer than 10 minutes to complete and is anonymous. If you decide to give your contact details to enter the prize draw or to stay in touch about future opportunities, these will not be linked to your results.
